Thank you for entrusting me with your holistic healthcare needs. Please complete the new patient questionnaire (NewPatientForms(HealthUnlimited).pdf) prior to your first appointment. 

If you are taking any nutritional supplements, vitamins, hormones, medications, protein powders, or applying any topical creams on a consistent basis, please bring them in with you on your first appointment as I will use these additional products in your analysis. 

You can expect the initial visit to be around 45 to 60 minutes. 

An additional request is that you bring a 2-3 day food journal with you, simply write down what you eat and drink in 24 hours for 2-3 days.  This is helpful, however, not mandatory.  I will cover foods and journaling on the first visit.
